NCH Patient Care Center – Colebrook (141 Corliss Lane, Colebrook)

Hours: Monday – Friday, 8 am – 5 pm

This facility is set up to meet our patient’s needs for non-emergency medical problems. Providers are specifically trained for comprehensive first contact and continuing care for patients with any undiagnosed health symptom or concern, as well as preventive care.

Pediatrics
Behavioral Health
D.O.T. physicals

NCH Patient Care Center – Stewartstown (6 Duranleau Street, West Stewartstown)

Hours: Wednesdays, 8 am – 5 pm

We encourage you to establish a relationship with a Primary Care Provider when you are in good health. This will ensure that we can provide you with the best care should you become sick or injured.
