Home health care is available to anyone in need of everyday assistance after an acute illness, injury or exacerbation of a chronic illness. Home health care providers help patients remain in the comfort of their own home rather than a more costly healthcare facility.  Services are provided through North Country Home Health & Hospice Agency.

Home Health is a Medicare-certified agency offering the following:

Hospice Care

Hospice is a centuries-old concept rooted in providing a place of shelter and rest to sick and weary travelers on a long journey. Hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible.

The hospice philosophy recognizes death as the final stage of life and seeks to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones.

The Hospice staff provides pain and symptom management, assistance with personal care, emotional support, and teaching care of the terminally ill, all with the goal of providing quality end-of-life care.

Medicare and most third party commercial insurers pay for Hospice services.

Hours of operation: Monday – Friday, 7:30 am to 4:00 pm

On call coverage: Monday – Friday, 4:00 pm to 7:30 am and all day Saturday, Sunday and holidays.
