It is our desire to allow patients to remain in their homes if that is their wish, for home is the best place of all to heal. We have a team of talented professionals that are experts at providing care at home and we are dedicated to fulfilling our patient's wishes and needs.
Our agency offers a variety of services to include:

- Skilled Nursing
- Follow-up care after hospitalization
- Physical assessments
- Medication review and teaching
- IV therapy in qualified cases
- Catheter care
- Newborn and post partum care
- Diabetic and Nutrition Education
- Physical Therapy
- Medical Social Worker
- have a newly diagnosed serious illness
- have a chronic condition such as diabetes, heart disease, multiple sclerosis, stroke, COPD, CHF, etc
- are disabled or frail and want to live independently but need assistance with bathing, meals or errands.
- have a child who is diagnosed with a serious illness or chronic condition
- had a change in medication and need some teaching to understand dosages, side effects, etc.
We cover 850 square miles to include the towns of: Atkinson, Gilmanton Academy Grant, Clarksville, Colebrook, Columbia, Dix Grant, Dixville, Errol, Ervings Location, Millsfield, Odell, Pittsburg, 2nd College Grant, Stewartstown, Stratford and Wentworth Location.
We are a Medicare Certified agency and also accept payment from: Medicaid, HCBC, Private Insurance, VA, Private Pay and offer a sliding fee scale for those who cannot afford but need the services.
